Home care nurses and clients are excited about remote care in Muhos

 

In January 2020, Video Visit® remote care was introduced in the municipality of Muhos, Finland, where home care nurses visit clients via video connection. 4000 virtual visits have already been made.

In remote care, the caregiver starts a video connection to the tablet at the client’s home. Marjatta Seppänen is visited both physically and virtually. Marjatta says that she has been a nurse and a nursing teacher herself. “I am familiar with eye drops and other procedures”.

 

Safety and care while living at home

 

The aim of the services targeted for the elderly is to support independent life at home by providing personal care, support and support services. The elderly care strategy is one of the key areas for the municipality of Muhos. Its mission is to provide the elderly population with the services they need, with a focus on preventive actions, while being founded on the true needs of the elderly as equal individuals (Municipality of Muhos website).

 

Sanna Paakkola, home care service manager, says that Video Visit’s video phone is used to ask about the client’s well-being, monitor medication intake, measure blood pressure and blood sugar, and guide meal time preparations. This way the client’s ability to function is being supported. She continues that the goal is to further develop, for example, stimulating activities, such as physiotherapist-led exercising.

“The home care nurse asks how I am doing and how I slept. The same questions that were asked on the phone before. Mainly the concern is as I am alone here, they want to be sure that everything is okay. The nurse also sees that I don’t talk nonsense”, laughs Marjatta Seppänen.

 

Tuula Niska, the director of elderly care at the municipality of Muhos, elaborates that being able to live safely at home is significant. “While cost savings are considered, I think that most importantly the content and quality of the service go hand in hand with that. Not everything can be measured with money. It is essential that the client feels safe at home”, says Tuula.

In Marjatta Seppänen’s home, there is a series of photos of her children and grandchildren. Marjatta’s daughter lives close and visits her mother. The boys live near Helsinki. Marjatta is very happy that she is able to stay connected with her family via video connection. She says that she is very delighted that she can see the growth of a small baby via video connection as well. Marjatta feels that her children do not need to be so worried about her because she has the tablet.

In particular during the coronavirus era, relatives appreciate that they too can stay connected with their loved ones via Video Visit’s video phone.

 

Successful start of a new service

 

According to Sanna Paakkola, service manager, the video phone diversifies home care services. “We are able to provide our clients with a much wider service array. Remote care is suitable for clients with different conditions, and it can even be used in the repatriation phase to support home care. Despite the challenging situation and lack of resources last spring, the staff has adopted the service well and their experiences have been positive.” Eija Kokko, remote care expert (Senior Specialist, Remote Care) from Video Visit praises home care nurses in Muhos: “I am very proud of them because despite the challenging spring, they implemented the service excellently”.

 

In Muhos, the aim was to invest in the introduction of remote care in order to ensure that the service would be implemented well. As a result, they introduced Video Visit’s professional service. “It has paid off – we have received a lot of help, satisfied clients and relatives”, says Sanna Paakkola. Remote care was introduced in the beginning of January this year, and the goal was to get 10 % of home care clients to start using the service. By having a good plan, being active and maintaining a positive attitude, the goals have been greatly exceeded. There has been no desire to reduce physical visits as remote care was meant to increase the provided services.

Kirsi Valli, service manager and elderly care professional, says that clients have been excited about Video Visit’s video phone, and the concept has been received well. A motivated team has encouraged the clients to start using it. “I am happy for the clients. Our staff has been very enthusiastic about this. Thanks to them, the clients are satisfied – which is a great thing”, says Kirsi.

Muhos invests in the future

Merja Honkanen, the director of social and health affairs, understood the significance of the start of the remote care project and provided an opportunity to receive help: without Eija Kokko’s expertise, the introduction of remote care would not have gone smoothly in Muhos.

Doctor’s appointment via chat helps to maintain the quality of the treatment – The city of Kalajoki is piloting a new service

 

The city of Kalajoki offers doctor’s appointments via chat for its citizens in primary healthcare. The new service model was developed in cooperation with VideoVisit Oy and Leluco Oy. During the piloting period, the service is free for the citizens, which continues till the end of September. How does the chat service work and what kind of feedback has been received?

 

The city of Kalajoki sought to implement a user-friendly chat function through which clients could easily and quickly have an online appointment with a doctor on weekday evenings, Saturdays and public holidays. It was also important that professionals working at the health care center would be able to consult a doctor via chat during business hours. Together with Leluco, VideoVisit developed a complete solution that includes both VideoVisit® Live chat technology and Leluco’s doctor service.

Easy and safe doctor’s appointment via chat

 

During the trial period, the citizens of Kalajoki and Merijärvi can visit a doctor virtually via chat free of charge till the end of September. A doctor’s remote visit is well suited for situations when physical examinations are not needed. For example, treatment assessments, health counseling, laboratory test result interpretations, and prescription renewals are possible via chat.

Access to doctors’ services becomes easier, even within a long distance. Clients can talk to a doctor via smart phone, computer or tablet. Conversation via chat is secure, and identification is done through suomi.fi service with online banking codes, mobile certificate or certificate card.

 

Five star service for citizens

 

According to Mervi Manninen, a health care center doctor in Kalajoki, introducing the chat function went smoothly – the start was clear, and it was easy to put into practice. The service has been used by professionals since April this year, and it was opened for local citizens at the end of May. During the piloting period, user experiences from the new service have actively been collected from both professionals and citizens in Kalajoki.

 

Mostly, the feedback has been positive. The overall rating of the local citizens who responded to the survey is five stars out of six.

Chat clients have also been asked to leave free-form comments and suggestions for further development. Mervi Manninen says that at the moment, the doctor’s chat is completely a text-based conversation platform in which professionals and citizens communicate by writing. “In the beginning, we wanted to test it as a text-based service, however, in the fall, the new updated version will take client feedback into consideration”, states Manninen. For example, the chat will allow the client to attach a photo of a skin issue.

 

The feedback from nurses has also been positive, and utilizing the chat has been perceived useful. The chat enables a quick contact, which has enhanced the process of reaching a doctor. “We had a broader development goal to improve collaboration between the nurses and doctors at the health care center. Previously, we were only able to consult our own doctor in the afternoon. With live chat, we can reach out to a doctor throughout the day as well”, explains Mervi Manninen.

 

Via chat, a doctor located in Tampere can consult a nurse, for example, on challenging questions that come from local citizens. “Remote doctor can present clarifying questions to the nurse without a delay, and the client gets an answer as quickly and accurately as possible. This way, the treatment quality is maintained” states Mervi Manninen when describing the benefits of the chat function.

Physiotherapy from workplace, home couch or sports field – Auron implemented VideoVisit® Live online appointment and consultation service

 

VideoVisit developed an easy and time-saving online appointment and consultation solution for the physiotherapists of Auron. “Good, now up. Turn around, back facing the camera, and then squat. There, good, and then up again”, says Minttu Vartia, a physiotherapist, as she instructs her client Timi Maunula via the mobile device’s video connection.

 

Timi Maunula injured his leg at a sports practice and had to undergo a surgery. He started remote physiotherapy because he wanted to recover quickly. Maunula says that before the remote physiotherapy sessions, he tried to rehabilitate his leg independently, but it was challenging, and he lost his motivation. He needed professional help and support in rehabilitation. Hence, he booked an online physiotherapy appointment with Auron.

“At Auron, we discussed how different providers’ solutions work together. We wanted to ensure that the client’s care path is as smooth and easy as possible”, states Ilkka Pirttimaa, the Chief Digital Officer of Auron. Auron’s physiotherapy services are available in 160 locations of which 61 are already part of a centralized online booking service.

 

Why partnership with VideoVisit was sought

 

“We wanted to find a solution in which the client can book an online appointment easily. This way, the computer does all the work in finding a physiotherapist that can offer service to the client in any of our available locations at any available time”, explains Pirttimaa. The client and the therapist both receive instructions and a link via email and text message to start the online appointment.

 

VideoVisit® Live met Auron’s needs as integrating it with Auron’s appointment booking service and patient information system was easy. VideoVisit’s mobile applications are suited for all platforms, which, according to Ilkka Pirttimaa, is important. VideoVisit® is secure and approved by Kela. This is one of the reasons why VideoVisit stood out from other competitors as Auron’s client base includes many Kela’s rehabilitation patients.

 

VideoVisit’s sales manager, Miitri Sinkko, explains that collaboration negotiations with Auron began at the end of 2019. “Right from the start, a picture of Auron’s advanced way of utilizing technology was conveyed, and a clear co-operation model was quickly formed. Collaboration with Auron is a great example of how to combine service business and technology expertise together with the aim of achieving the best possible customer experience”, he continues. The well-made plans were put into effect on an accelerated schedule in March.

 

The benefits of remote physiotherapy

 

Via VideoVisit® Live service, Auron’s clients are able to do exercises and rehabilitation under the guidance of a physiotherapist at home, in their own familiar environment. Remote physiotherapy via a video connection speeds up the access to this service as available physiotherapists from various locations can serve clients. Online appointments are also cheaper, and they are possible via a mobile device or tablet.

 

According to physiotherapist Minttu Vartia, VideoVisit® Live service is a good option for those clients who have difficulties in coming to see the physiotherapist in person. “We have a lot of clients in risk groups. For example, people over the age of 70 with a medical background. There has to be a way to offer physiotherapy in every situation”, states Vartia.

For Timi Maunula, due to the coronavirus situation, online appointments were the only way to receive physiotherapy. It enabled treatment and guaranteed continuity. In order to ensure the proper healing process, it was important to begin the treatment right away. “Rehabilitation should take place at the same time when the tissue heals. It is very important in rehabilitation after a surgery”, emphasizes Minttu Vartia when describing the healing and rehabilitation process.

 

VideoVisit® Live solution enables smooth and non-location bound appointments. The physiotherapist utilizes the mobile device to show the client which equipment to use and how to do different exercises. Traditional physiotherapy includes manual treatment which means treating the tissue by using hands. In remote physiotherapy, the physiotherapist can instruct the client to perform different treatments and exercises by themselves. “Initially, physiotherapists were required to change their mindset and understand that physiotherapy can be performed effectively without  touching the skin”, states Minttu Vartia.

 

Via video connection, each physiotherapist is able to practice their own way of working. “A physiotherapist’s own personal style is reflected on the video – what they pay attention to, what kind of contact they make with the client, and how they manage the situation”, Minttu Vartia clarifies. “For example, a good and close client relationship was formed with Timi already after a couple of sessions. And we have only met via video connection”, Vartia states.

 

Coronavirus increased the amount of online appointments

 

In March of this year, the effects of the coronavirus started to be fully realized. Within a few weeks, many different service providers introduced remote services on an accelerated schedule, and daily in-person visits were turned into online appointments. “The number of professionals logging into VideoVisit service increased substantially during March and April”, states Miitri Sinkko. VideoVisit services are easy to use and reliable, which enabled quick adoption of new operating methods.

In the city of Oulu remote care is perceived more pleasant than the traditional form of home care

 

Remote care complements home care visits of the city of Oulu and enables wider and more diverse services. The city’s remote home care has been ongoing since June 2019 via a secure video connection. The remote home care clientele receive a tablet to their home, and the connection starts when the caregiver calls.

 

The aim of home care of the city of Oulu was to find a solution for everyday life that would replace some of the physical home care visits. After introducing the VideoVisit® Home Virtual Care Service, physical home care visits have been able to target those clients who need services and the caregiver’s help at home. Remote caregivers became responsible for visits that do not require physical contact.

 

Sirpa Saarela, the city’s coordinator for the development of welfare services, states that examples of implementing remote home care are found in other cities. The Virtual Home Care service produced by VideoVisit Oy, or more familiarly called the video phone call service, is currently being utilized in Soite, Kuusamo, Tampere, Turku, Eksote and PHHYKY. According to Jarkko Kukkonen, the account manager of VideoVisit, remote home care visits have been on the rise for several years. However, in the spring of 2020, the number of users increased significantly due to the situation caused by the coronavirus. “All signs indicate that remote services are here to stay, including remote home care provided by the municipality”, Jarkko summarizes.

 

Remote home care is well suited for monitoring the client’s daily activities. Caregivers remind clients, for example, to take medication and eat. In addition, blood glucose measurements and insulin injections are monitored through video connection. During the remote visit, there also is some time for catching up on daily events.

 

“The client does all the daily chores at home, and the caregiver guides them. This type of home care maintains their ability to function better because the caregiver does not do things for the client”, states Sirpa Saarela when describing the rehabilitative feature of remote home care.

 

Monthly goal of 3000 virtual visits about to be exceeded

 

The remote home care of the city of Oulu is exceeding its monthly goals. The targets were already achieved at the end of 2019. Last December, as many as 1700 remote home care visits were made in Oulu. Sirpa Saarela says that this year’s goal is to make 3000 remote home care visits each month. “Now we have almost reached this goal although there is still some time left in May”, Saarela continues.

 

Positive client feedback

 

According to Sirpa Saarela, remote home care has started off very well, and according to the feedback received from the clients, remote care is perceived as even more pleasant than the traditional home care. “The caregiver visits the client via video connection and reminds them about things that are important in their daily lives. Clients feel that this circumstance is more comfortable than a situation when a stranger physically arrives at their home”, she clarifies.

Anne Terho, who works as a nurse, also agrees. She feels that VideoVisit® Home service calms the client’s life at home. “It means that there are not people coming and going all the time, and sometimes out of four visits, three visitors are different caregivers”, she continues. Anne delivers the devices to the clients and instructs them how to use the tablet. On the first visit, a test call is made in order to ensure that the connection works. According to Anne, the first reaction has mostly been positive, and it has been nice to deliver devices to clients. “They are happy that they do not need to know how to use the device. The most important thing is that the connection is activated by the caregiver”, states Anne. She is delighted to notice that even those clients who had their doubts in the beginning have started to enjoy video calls with their caregivers.

 

Currently, remote home care of the city of Oulu has about 130 to 140 clients. All of the city’s remote home care client visits are made centrally from the same location: Home Living Support Centre (Kotona asumisen tuen keskus, Kotas). Sirpa Saarela says that new home care clients are generally referred to a 2 to 8 week rehabilitation period before they become actual clients of home care. During the rehabilitation period, it is identified which clients need physical visits and which are suitable for remote home care. Sometimes physical visits turn into remote visits after the rehabilitation period. You can also become a remote home care client without the rehabilitation period depending on your needs for the service.

With VideoVisit® Home Virtual Care Service it is possible to serve a wider client base centrally as distance no longer matters. “When the connection ends the caregiver can immediately call the next client regardless of where in the city of Oulu the client lives”, says Saarela. Joel Holmén started as the Medical Advisor of VideoVisit Oy on 1 st of April 2020. He is a doctor who specializes in psychiatry and also a doctoral researcher who has worked several years with geriatric psychiatry at the Turku University Hospital. Joel has acquired an extensive expertise in geriatric mental health care as well as using technology to support treatment.

 

The doctor from Turku is familiar with online appointments. Joel Holmén has taken part in developing an online appointment service for the Geriatric Psychiatry Unit of Tyks in which VideoVisit® HOME Virtual Care Service was introduced in early 2019. In Southwest Finland, elderly people often live in remote areas and have difficulty accessing, for example, municipal health care services. Remote home care is a way to speed up the hospital discharge process and provide more effective services to a patient’s home. Mental health problems of the elderly are often overlooked. Good mental health is a prerequisite for human well-being. Joel Holmén says that in Finland society is not prepared for mental health challenges of the elderly, such as the treatment of depression. Access to mental health care in primary health care has decreased and resources have been reduced. Too many elderly people are left without the care they need. For example, Kela (the Social Insurance Institution of Finland) does not provide reimbursement for rehabilitative psychotherapy treatment of elderly people over the age of 67 years as mental health services are targeted at young people and people of working age. “It has been studied that psychotherapy is beneficial for the elderly. Depression is often driven by the feeling of loneliness,” says Holmén.

 

According to Joel, untreated mental health problems of the elderly are reflected in home care and other services used by the elderly. It manifests itself in greater number of more severely symptomatic patients. The quality of life of the elderly could be improved by providing low-threshold services and improving access to and the speed of care.

Specializing in geriatric psychiatry, Doctor Holmén is concerned about the mental health of the elderly. The elderly population has been the most severely affected by the isolation guidelines due to the epidemic. The elderly people have been prohibited from relatives visiting and therefore their human contact has drastically reduced. The lives of both those living at home and those living in service homes have become more lonely. Many miss connecting with family and friends.

 

The state of emergency causes worry, sadness, fear, insecurity and anxiety. Isolation, impaired communication with both caregivers and loved ones, and the lack of presence affect mental well-being.

The UK uses technology against the coronavirus – chooses Finnish VideoVisit to support the elderly isolated at home

 

In March, the UK Department for Health and Social Care (DHSC) and The Ministry of Housing, Communities and Local Government (MHCLG) announced that funding would be available to innovators who can find digital ways to support people who need help during the coronavirus (COVID-19) outbreak. This ‘Techforce19’ challenge aims to support those who are particularly vulnerable or isolated as a result of the outbreak and need help, including people who need mental health support or who have social care needs.

 

The NHSX, which is responsible for the digitalization of public social and health services in the United Kingdom, has been working with partners PUBLIC and the AHSN Network to run the fast-track competition for innovators, creating a pool of ready to go technologies that have the potential to be rapidly scaled regionally and nationally.

 

Through ‘Techforce19’ challenge, NHSX chose Finnish VideoVisit® HOME remote care platform to support the daily lives of elderly people who have been isolated at home because of the coronavirus.

 

In the application process, 18 different solutions were selected from among 1 600 service providers. VideoVisit was the only solution chosen from outside the UK. With the help of the remote care platform, the NHSX wants to support communication in particular between the vulnerable elderly, their relatives and caretakers. With VideoVisit, it is possible to make home care visits via a secure remote connection. In addition, elderly persons using the service will be able to have video connections with their loved ones.

 

According to VideoVisit’s CEO Juhana Ojala, the strength of the service lies not only in its ease of use, but also in its humane dimension.

 

VideoVisit cannot be compared to traditional telemedicine or conference services, for example. VideoVisit is a door to the world for the person taken care of at home. It brings together the nursing staff, the client living at home, who needs regular care, and his loved ones.

VideoVisit is already Finland’s leading remote care service platform. In Finland, the easy-to-use service designed for the elderly is already in use in half of Finland’s municipalities and with thousands of home care customers. The possibility to join the TechForce-19 project will open an important route for VideoVisit to the UK market, where the company works closely with locally based Nursebuddy.

 

Nursebuddy, a key partner for VideoVisit on TechForce19 initiative, has established its position as a trusted solution not only in Finland but also in the UK and Ireland. Nursebuddy enables homecare managers to plan and manage visits effortlessly remotely. The aim of the business collaboration is to expand the remote care service to Nursebuddy’s customer organizations in the UK, and to further integrate both solutions together.

 

The coronavirus has increased the need for remote care for the elderly to a whole new level. Home care service providers are deploying services at a record rate to ensure the availability and quality of their services. In order to minimize the spread of the coronavirus to the at-risk group, as many visits as possible are transferred to remote visits. Similar developments are taking place throughout Europe.
